Is physiotherapy good for sciatica?

Physiotherapy is a first-line treatment for most sciatica. It reduces nerve irritation through positioning and movement, restores mobility, and rebuilds strength and tolerance. Most sciatica improves without surgery, though the timeline varies and severe or progressive cases need medical review.

What sciatica actually is

Sciatica describes pain travelling along the sciatic nerve — usually from the lower back or buttock down the back of the leg. It is a description of symptoms, not a diagnosis in itself, and the underlying cause is most often irritation or compression of a nerve root in the lower spine.

Symptoms

  • Sharp, burning or shooting pain down one leg
  • Pain that worsens with sitting, coughing or sneezing
  • Pins and needles or numbness in the leg or foot
  • Weakness in the leg or foot
  • Pain typically affecting one side

What treatment may include

  • Assessment to confirm the pattern is nerve-related and identify the level involved
  • The specific positions and movements that ease your symptoms (these differ between patients)
  • Nerve mobility work and manual therapy
  • Progressive strengthening once symptoms settle
  • Dry needling where clinically appropriate

When to seek urgent medical care

Seek immediate assessment if you develop loss of bladder or bowel control, numbness in the groin or saddle area, or worsening weakness in both legs.

Frequently asked questions

How long does sciatica last?

Many cases improve substantially within six to twelve weeks, though some resolve faster and some take considerably longer. Persistent or worsening symptoms warrant medical review.

Will I need surgery for sciatica?

Most people do not. Surgery is generally considered when symptoms are severe, persist despite conservative treatment, or when there is progressive neurological weakness.

Should I stretch if I have sciatica?

Not necessarily — aggressive stretching can aggravate an irritated nerve. Which movements help depends on your presentation, which is what assessment establishes.
